When people start regaling onlookers with tales of your early chronicles, and you can’t even remember the tour of duty… that’s a pretty good indicator that your career is pretty much over. Time to find meaning elsewhere and let more capable folks take the helm.

John Akers – I should have added, this was 15 years ago.
Bruce had a one-hour meeting with them. That’s a very, very short tour of duty.
For those in professional services consulting, there are hundreds of meetings a year. Expecting someone to remember details from a meeting that long ago is not realistic.

“Underpaying for the role” Especially in hourly, 1099, positions.
Since I started responding to recruiters with what I’m making now, adjusted for a 1099 (at the very least triple what your hourly take home is), the number of recruiters spamming me has declined quite a bit.
“Not understanding the needs for the role”
“And just where on my resume did you see any indication I have X years of experience in areas related to role Y?” is often followed by “Ummm. Well…”
